If you are a new patient, please complete this patient onboarding intake form fully. Feel free to give as many details as you like and list all of your symptoms, aches and pains. This form must be filled in before your first appointment.
You can amend your appointment yourself via the clinic’s online booking system.
How to get to your appointment
The clinic is located at 1A 713 Main Street in Canmore. Upon entering building 713, walk down the stairs and the clinic door will be there on your left.
